Clinical depression increases cardiac arrest risk

 

 

 

Study findings suggest that clinical depression may increase the risk of cardiac arrest independently of established coronary heart disease risk factors.

J Empana (H�pital Paul Brousse, Villejuif, France) and colleagues found that clinically depressed patients were almost twice as likely to suffer cardiac arrest as non-depressed individuals.

The association was observed in both genders, in various age groups, and in patients with and without a history of heart disease.

"Alteration of the imbalance between sympathetic and parasympathetic tone has been described in depressed subjects, yielding the possibility that this population may be at particular increased risk of sudden cardiac death," note the researchers.

To assess whether this is the case, the team reviewed data from a population-based case-control study of risk factors for incident out-of-hospital cardiac arrest.

Participants included 2228 patients aged between 40 and 79 years who experienced cardiac arrest between January 1980 and December 1994, and 4164 people who did not suffer a cardiac arrest. Of these individuals, 1402 and 2239 from each group, respectively, had a history of heart disease.

Depression, defined as a physician diagnosis or use of antidepressant treatment within the year before the event, was seen in 300 of the patients who experienced cardiac arrest and 327 of controls.

Overall, clinically depressed patients were 1.88 times more likely to experience cardiac arrest than controls.

This association persisted after taking into account coronary heart disease risk factors, including cigarette smoking, heavy alcohol consumption, diabetes, hypertension, prior myocardial infarction, and prior congestive heart failure, with an odds ratio (OR) of 1.43.

Moreover, the team found that the risk of cardiac arrest increased in accordance with depression severity. Compared with participants with no depression, the risk of cardiac arrest was increased 1.3-fold in those with moderate clinical depression and 1.8-fold for those with severe clinical depression.

Commenting on what mechanisms may underlie the association between depression and cardiac arrest, Empana et al say that atherosclerosis or alteration of the cardiac autonomic response may play a role. Alternatively, poor adherence to treatment may be a contributing factor.

They conclude in the Archives of Internal Medicine that, if replicated in other studies, their results "may have potential clinical implications with regard to risk stratification for out-of-hospital cardiac arrest."
